Issue Statement: In 2012, the City approved a medical director agreement with Dr. John Cunha with an initial term of two years and two optional two-year renewals.  Under the previous contract with Dr. Cunha, these renewal options were exercised in 2014 and 2016 and the contract is set to expire at the end of November.  As no further renewal options remain on this contract, in June of 2018 the City issued a Request for Proposals (RFP) to secure a contract for Medical Director Services.

Recommended Action: The City Commission adopt a resolution awarding the medical director services agreement to the most responsive and responsible proposer, Dr. John Cunha.

The City of Oakland Park Fire Rescue Department's Emergency Medical Services License for Advance Life Support requires a Medical Director in accordance with Florida Statue 395/401,  Department of Health Rules and Regulations 64J-1 and Broward County Code Chapter 3.5.

On June 29, 2018, the City advertised RFP #072718, soliciting proposals for the purpose of providing Medical Director Services to the City’s Fire Rescue Department. Sealed proposals were received until 2:30 PM on July 27, 2018, and responses were opened shortly thereafter. The City received two proposals:

• Dr. John Cunha
• Dr. Antonio Gandia

On September 17, 2018, the Evaluation Committee (Chief, Krivjanik. Fire Fighter / Paramedics, Chris Lunt, Kraig Zauner and Myles Anderson,) met to discuss, review and score the proposal responses based on the criteria as explained in the RFP.  Dr. John Cunha's proposal received the highest score and is the recommended proposer of the evaluation committee. The contract's initial term would be for two years, from December 1, 2018 to November 30, 2020, with three (3), two (2) year extensions upon the mutual agreement of both parties.

Funds are included in the FY19 Operating Budget of the Fire Rescue Department for the Medical Director Services under the professional services account.  As shown in the vendor proposal of Dr. J. Cunha, D.O. and as referenced in the attached contract for services, the annual cost is $26,400 which is to be billed and paid by the City on a monthly basis.  The proposed expenditure is as follows:
